Stroke Survivor Connection brochure

The Stroke Survivor Connection is a free program of physical and mental activities and counseling to support patients and their families recuperating from stroke. The Stroke Survivor Connection is centered on weekly meetings, hosted by trained facilitators. The meetings offer mental and physical exercise, peer support, sharing of information and resources, and valuable socialization opportunities. The program offers connection to friendship, fun and support when it's needed most. It's priceless...and free of charge.

Unfortunately, due to funding difficulties, Health Hope Network has had to suspend its sponsorship of the Stroke Survivor Connection program. However, several group leaders, sponsors, and sites have kindly stepped up to continue the program on their own, most on a biweekly or monthly basis. Please see the list below for locations and contact information for the remaining groups. Please contact the group leader or site directly for more information about attending a group. Health Hope Network is currently searching for funding and/or a new organization to sponsor some or all of the groups.
